Based in Western Australia, Shukran Services aims to provide supports to individuals with special needs. Our trained experts provide the necessary assistance needed to live independently and enjoy unhindered access in the community. Respite Services

At Shukran Services we provide respite supports for people seeking a place to live, which suitably caters to the type and level of needs required. Assistance with daily personal activities to enable people to live as autonomously as possible – such as personal hygiene, dressing, medication assistance, assistance with budgets, communication skills and lifestyle mentoring.

Support Care with Daily Personal Activities

We provide support for all funding categories:

Core
Core supports are designed to assist day to day living and can also be used for social, community and transport services.

Capacity Building
This is person led funding to pursue your interests in developing participants existing living skills and learn new ones. Goals may vary from employment, develop relationships, or improve health and wellbeing.

Daily Tasks Assistance

Our support staff assist with daily tasks in group or shared living arrangements to help individuals to live as autonomously as possible. As part of the supports we look at life stage transitions, including mentoring, peer support, individual skill development, and decision making to support people to make transitions on their own terms and in a way that meets their goals and aspirations.

Employment or Education Assistance

Support to access and maintain employment or higher education – assistance and encouragement with preparation for school leavers to find employment in either the supported or open job market, and in their transition through school towards further education.

Group and Centre-based Activities

We at Shukran facilitate Group and Centre-based activities such as social outings and group and social activities Participation in the community, including supported shopping, medical appointments, sporting and recreational events, social activities, visiting or making new friends, travel and transport and building confidence and social skills.

Specialised Positive Behaviour Support

Our Positive Behaviour support services include assessment, development of a plan aiming to limit the likelihood of behaviours of concern developing, or increasing Specialised Supported Employment – provision of assistance with disability employment, including Australian Disability Enterprises (ADES)

Support Coordination

Support coordination is a capacity building support service that assists a participant achieve their goals by improving their understanding and better navigating their NDIS plan. This is achieved through connecting participants to NDIS providers and therapists, assist to independently maintain or improve current supports received and get the most out of your plan. Our support coordinators will follow up with service providers, attend meetings and prepare you for your plan and review.

Services for NDIS Participants

Are you an NDIS participant?

At Shukran we operate on a person-centred model where you are at the centre of the decision-making. Our support staff will work with you and your family to reach your goals.

Our services are person centred, holistic and respond to the needs and goals of our participants, their families and carers. We listen to you, and in partnership with you, we develop personalised plans and supports that ensure that you receive the kind of supports you desire.

DAILY TASKS

Supporting you to develop the skills you need. We can also provide respite care, day or night, to allow your carer to take a break, study, work, or go to appointments.

COMMUNITY & CENTER-BASED ACTIVITIES

Providing meaningful social, recreational and skill growth activities in a group setting either in the community or at a local facility.

HOUSEHOLD DUTIES

Our cleaners and gardeners can come to your home and assist you with the jobs around the house and make sure your home is cleaned, as you require.

TRANSPORT

Do you require transport as part of your NDIS support? We can help with your local community connection.

Shukran Services and its workers enforce the necessary practices to adhere to NDIS Practice Standards and the NDIS Code of Conduct. This is done to ensure Shukran Services and its workers are able to effectively promote and provide quality care for all participants.

Shukran Services strives to provide expertise that fits established and planned customer requirements. That involves taking into consideration the participants’ cultural background disability and different communication requirements.

If participants or stakeholders seem to be dissatisfied with any element of accessibility of the provider, they are directed to the Shukran Disability Services policies and procedures for feedback, compliments and complaints.

SERVICE ACCESSIBILITY CONSIDERATIONS

Our Services understands and recognises that some participants may be physically unavailable to access Shukran Services’ services. If this situation arises, Shukran Services will be sure to find alternative strategies to provide these participants with the best possible care with the certain restrictions. Workers need to seek alternative strategies for example:

  • Having over-the-phone meetings

  • Having skype calls

  • Meeting the participant within an accessible community premises such as a library or café.

  • Hiring an accessible fee-for-use serviced office in which meets the standards.
